Northeast Georgia Diagnostic Clinic uses an electronic medical record. The process of requesting copies of your medical records is as follows:

Once the request form is complete and signed you may submit it to NGDC one of the following ways:

All Release of Information requests are processed by our third-party vendor, HealthPort. HealthPort handles the delivery of all records. Please note that a per page charge may apply to some requests (in compliance with Georgia law). The per-page charges are: .97¢ per page for pages 1-20; .83¢ for pages 21-100; and .66¢ for pages greater than 101.
Records that are being sent to another provider directly from NGDC will be processed at no charge to you. All other request will be billed appropriately.
You may choose to receive your records either via mail or electronically. If you choose to receive your records electronically, please ensure that you provide your complete email address on the release form. When your records are ready, you will receive an email with a link and a password that can be used to access your invoice and records. You will then be able to save, print, or forward your records as appropriate.
Release for Radiology Disk
If you are requesting a radiology disk, these will be processed by the Radiology Department. Please allow a 24 hour notice for all radiology disk requests. In the instance that you need a personal disk or additional copies, a disk fee of $40.00 will apply per disk. Radiology disk fees will also be invoiced and collected by HealthPort
Requesting Forms to be Completed
In order to have forms completed by NGDC, you will need to fill out a Release of Information Request form and present with forms. Please note a $25.00 charge may apply to some form request.
All records requests are prioritized and processed as quickly as possible. Please allow several business days for us to process your request and get records out to you.
