TO SCHEUDLE A PATIENT FOR A SLEEP STUDY:

1)      Complete the Polysomnogram Order form

            Including:  clinical symptoms and special needs

2)      Have patient complete Epworth Sleepiness Scale

3)   Call Sleep lab Scheduling office to schedule 770-297-4985

4)      Fax Order, Epworth, pt demographic information, Physician’s office notes (indicating need for study), and list of medications and allergies to 770-297-5020

5)      If you have questions, call the Sleep Center at 678-450-3625.

A packet of pre study instructions will be mailed to your patient from the Sleep Lab.
