Mission
The Chronic Kidney Disease
Center is dedicated to providing the highest
quality
care to our patients as we work to prevent adverse renal
consequences
and slow the progression of chronic kidney disease.
About
Us
More than 20 million
people in the United States, one in nine adults,
have
Chronic Kidney Disease (CKD) and most of them do not know it! CKD
does not usually give any
symptoms until it is quite advanced. Therefore,
early recognition and treatment are essential.
The
CKD center at Northeast Georgia Kidney Center was developed in
order to help improve care for
CKD patients in the northeast Georgia
area.
This center offers a comprehensive multi-disciplinary approach to
CKD patient care. Through CKD
monitoring, nutrition counseling, anemia
management,
and patient education, we work with the patient’s primary
physician to prevent adverse
renal consequences and to slow the
progression
of disease. Our scope of care also incorporates the
treatment
of comorbid conditions including hypertension and hyperlipidemia.
Northeast Georgia Kidney
Center’s multi-disciplinary team is made up of
physicians,
nutritionists and nurses who collaborate to improve the
care
of and outcomes for patients suffering from kidney disorders.
Services extend from the point at
which there is slight kidney damage
but
normal function to the point at which patients require renal
replacement therapy.
We value the importance of the
primary care physician-patient
relationship
and always work to include the referring physician and the
patient as part of our care
team. We will continually work to
maintain
open communication and mutual respect among all members of a
patient’s clinical team and
are committed to developing healthy, on-going
relationships with area physicians.

mildest form of kidney disease (Stage I) to the
most advanced (Stage 5).

epoetin alfa administration
We provide these services as
requested by the referring physician and
we
work diligently to communicate the CKD care plan to all physicians
involved.
The
Multi-Disciplinary Approach
Working closely with the
referring primary care physician, our
nephrologists
will establish a diagnosis and provide a patient
assessment.
Following a battery of tests, we will develop a patient
care
plan to share with the PCP who will oversee and manage the care
directly with the patient. The
care plan routinely includes periodic
monitoring
of blood pressure, Hb level and testing of other parameters
such as serum albumin,
phosphorous and PTH levels. If the patient
fails
to achieve target levels, the Northeast Georgia Kidney Center
may need to reevaluate the
patient or make new recommendations to the PCP.
Our nutritionists, nurses and
physicians work to empower patients to be involved
in their own self-care.
In case of advanced kidney
disease, the CKD Clinic helps and prepares patients
and families for renal replacement therapy.
