A Team Approach to
the Treatment of Diabetes
Northeast Georgia Diagnostic Clinic offers a comprehensive Diabetes
Education Program which is recognized by the American Diabetes
Association. The Clinic also offers the services of two full-time and
one part-time Certified Diabetes Educators for patients to help them
learn to manage their diabetes.
 The Diagnostic Clinic uses a collaborative approach in planning your
diabetes care.
In addition to seeing your personal physician, you may
see one of our board certified Endocrinologists, Matthey Harris MD
(left) or Cacia Soares-Welch, MD, (right) or have them
consulted by one of our educators
about your care. If you are experiencing kidney failure or need transplant follow-up care, you may also see one of our nephrologists.
And the team goes beyond our physician specialists
to include:
Mitzi Lovell, RN, CDE, (left); Amy Roark, RD, LD, CDE, (middle);
and Jennifer Roark, RN, FNP,
CDE, (right) our Certified Diabetes Educators who will help you learn how to effectively
manage your diabetes.

 
Diabetes Self Management Education
An initial Assessment, 5 classes and quarterly follow-ups. Include
SMBG, Carb counting, heart healthy meal plans, education on
medications, complications and management of diabetes.
Pre-Diabetes Class
A one time class for patients diagnosed with Pre-Diabetes. You will
receive a free glucose meter, pedometer and will be instructed in
the lifestyle change recommendations of the Diabetes Prevention
Program.
Individual Nutrition Consult
Patient's current nutrition status and eating habits will be
assessed and a plan formatted to meet patient/health
care provider's goals. Patients can be seen for Weight Management,
PCOS, Cardiovascular Disease, Hyper-tension,
GI Disorders, food allergies, etc.
ON YOUR WAY Program
A
lifestyle change program designed to help patient's deal with eating
issues.
Patients can join and drop out at any point in the program, however,
our Dietitian recommends an 18 month commitment, in order to give
patient's the time they need to put new behaviors and thought
process in place, and to practice these new ways of life with
supervision when challenges arise.
CKD Education
An initial Assessment & 2-3 follow-up classes related to specific
CKD stages.
Heart Healthy Class
A one time class discussing the Lipid and BP targets and dietary
methods for reaching the targets.
RMR Test (Medgem) & Interpretation Class
A 5-10 minute breathing test which
measures the Resting Metabolic Rate. The test can give us the
missing information we need to help patients with weight change.
They will receive the interpretation of their test in a group format
& can choose at that point if they would like more intensive
counseling or assistance with putting the information to work for
them.
Insulin Pump Training
For patients who would benefit from an insulin pump, this may be the
route to choose. They will be thoroughly assessed for readiness and
necessary management skills.? They will receive the education needed
to fill in the gaps. Their paperwork will be taken care of as well
as the set up for placement on the pump.
Continuous Glucose Monitoring
System (CGMS)
If you have erratic and difficult to control blood glucose, this is
an opportunity to be monitored for 3-4 days via an implantable
sensor. This test allows for detection of lows and highs in those
who may have lost their own ability to sense BG swings.
Prednisone Class
For people new to steroid therapy or those who have been treated
with steroids for some time. Come learn about how to protect your
eyes, bones, waist line, blood glucose & blood pressure from the
affects of steroids.
